Information below on orchiectomy, vulvoplasty, and vaginoplasty:

Orchiectomy

Orchiectomy is a surgery to remove the testicles. This can be done as a stand-alone procedure, prior to any other reconstruction, or at the same time as vaginoplasty/vulvoplasty.

Vulvoplasty

Vulvoplasty is a surgery to construct the external feminine genitalia (vulva) This includes the labia, a sensitive clitoris and lower-positioned urethra.

Vaginoplasty

Vaginoplasty is a surgery to construct the external feminine genitalia (vulva) and an internal space (vagina). This includes the labia, a sensitive clitoris and lower-positioned urethra.

There are two options for canal construction: 1) using current genital skin (“penile inversion”) or 2) using a combination of genital skin and inner belly lining (peritoneum)

There are additional options should they be a better fit that can be explored with your surgeon including scrotectomy, nullification, and penile sparing vaginoplasty

 

What Can I Expect Prior To Surgery?

Complete an intake form that allows us to get to know you and your current urinary/sexual function status

You will meet with your surgeon and review your surgical options, risks, and benefits of the procedure, and perform a physical exam

You will obtain an education booklet that reiterates what was reviewed during your consultation. (insert link)

You will meet with the surgical team several times to ensure all of your questions are answered and you feel vomfortable with the plan.

 

What are the specific pre-surgical requirements

  1. Permanent hair removal is a requirement before vaginoplasty since hair bearing skin is used to line the internal vagina. This can be accomplished with either laser or electrolysis. In order to prevent difficulties with dilation or infection risks we require 100% hair removal.
  1. Assessment letter by a gender affirming health provider is another requirement before any bottom surgery can be performed. We follow the WPATH guidelines which recommends only 1 assessment. However, some insurance companies still require 2 letters from a mental health provider, so be sure to check with the requirements of your insurance.
  1. Medical optimization is critical prior to scheduling surgery. We want to make sure you are healthiest safe and minimize any potential complications. This may include smoking cessation, ensuring your diabetes is well controlled, and managing any major medical problems.
  1. Every surgical candidate meets with our social worker who helps to ensure there are no non-medical needs that need to be met prior to surgery

How Long Is Recovery?

Recovery can vary depending on the type of surgery you’ve had. Orchiectomy is an outpatient surgery with a 3-4 week recovery.

Vulvoplasty and Vaginoplasty are inpatient surgeries in which you can expect to be in the hospital from anywhere from 3- 5 days. There is an expected 6-8 week recovery in which you will be asked to take that time off school/work/etc.

Follow ups after vaginoplasty will be weekly for 4-6 weeks to ensure you are healing correctly and to ensure you are comfortable with dilation.
