Patients insured as a dependent on someone else's insurance, may be able to prevent the insured person from receiving information about care received. Contact your insurance company and ask about privacy options under the California Confidentiality of Medical Information Act (CMIA).

Identity Documents
Changing your legal documents to reflect your identity:
- Changing your name only requires a court order. No physician letter is needed.
- Changing your California birth certificate does not require a physician letter or court order. Other states may have different requirements for birth certificate changes.
- Changing your gender marker on your California state ID or driver license does not require a physician letter. Other states may have different requirements.
